Statement on Discrimination and Harassment–Approved by the Office of Equal Opportunity and Compliance, university legal counsel, and the EVPP

It is strongly suggested that every course syllabus include a statement addressing the responsibilities of faculty and staff in relation to students with disabilities. Here are few suggested disability statements:
"If you have a documented disability and anticipate needing accommodations in this course, please make arrangements to meet with me soon. Please request that a Disability Resources staff send a SAAR form verifying your disability and specifying the accommodation you will need."
“If you have a disability and require accommodations, please contact the instructor early in the semester so that your learning needs may be appropriately met. You will need to provide documentation of your disability to the Disability Resources (DR) office, main floor of the Students Services Building, Room 1076, 515-294-7220.”
